ディスクレパンシーの診断がギリギリの症例のおいて、抜歯の必要性を検討することは難しい。
成人患者はいい歯並びだけではなく、顔全体の美的改善も求める。ほとんどのアジア人患者はアライナー矯正治療からより小顔になることを希望する。数多くの矯正医は側貌に著しい変化がみれる治療は抜歯のみだと考えている。だが、特に薄い軟組織側貌のある成人患者において不必要に抜歯を行うことで、 非審美的なガミースマイル、凹みすぎる側貌、老けて見えることなどの恐れがある。本講演は審美的な顔面バランスを重視した様々な非抜歯症例を発表する。
Treating adult patients with borderline space discrepancy can be somewhat confusing and needs a sound judgment whether or not an extraction treatment plan is really needed. Adult patients often demand not only straight teeth, but also an improvement in overall changes in facial esthetics. For most Asian patients, facial tapering is one of the most desirable outcomes when seeking orthodontic treatment including clear aligner therapy. As majority of clinicians have impression that the way to deliver marked change in facial profile often involves some tooth extraction. However, an extraction treatment done unnecessarily can result in dished in anterior teeth, unesthetic gummy smile, too concave facial profile, and aged-look appearance, especially in adult population with thinner soft tissue profile and existing bony facial appearance.
This presentation will showcase a variety of aligner orthodontic treatments with a more conservative non-extraction approach with focus on the facial esthetics and harmony of the face. A good number of non-extraction treatments with favorable and optimal change in facial esthetics will be presented and discussed.
Thiti Sirikrai, DDS, MS
Dr. Thiti Sirikrai obtained his dental degree from Chulalongkorn University in 1996, Certificate in Clinical Orthodontics, and Master of Science in Orthodontics from Saint Louis University, Missouri, USA in 2000. He has been a visiting lecturer in Biomechanics and Invisalign for Postgraduate Orthodontic students at Chulalongkorn University and Mahidol University in Bangkok since 2007.
Dr. Thiti Sirikrai is a member of the American Association of Orthodontists and World Federation of Orthodontists. In 2002, he became an accredited Invisalign provider and obtained Diamond Provider status in 2014. Up til now he has completed more than 1,600 Invisalign treatments. Dr.Sirikrai is also an Invisalign Speaker and has conducted several Invisalign Certification courses and Study clubs in the Philippines, Vietnam, Indonesia, India, Thailand, Singapore and Hong Kong. Now Dr.Sirikrai serves as Associate Clinical Director at Thantakit International Dental Center, his family own private practice in Bangkok since 1945.
