< Meet your success in Class II treatment with Invisalign > - Abstract: As the proverb says “Prevention is better than cure” this should be applied to orthodontics in order to reduce the severity of the future malocclusion and and to provide an early correction of the skeletal abnormality. Orthodontics as a branch is more concerned with facial growth, development of dentition, and occlusion. Aligners were not able in correcting dentoskeletal anomalies since few months ago, when Align Technology (San José, CA, USA) introduced the Mandibular Advancement feature. Functional appliances are widely/used in orthodontics for skeletal Class II correction, i.e., advancement of the mandible and the same principles of well known functional appliance (Twin Block) were applied to aligners in order to intercept class II malocclusione related to mandibular deficiency. Preliminary result related to the treatment of growing patients will be presented.
The distalization of maxillary molars is frequently required in class II non-extraction patients. Resolving class II molar relationships by distalizing maxillary molars may be indicated for patients with minor skeletal discrepancies. The upper molars can be distalized by means of extra or intraoral forces. In recent years, several techniques have been developed to reduce the dependence on patient compliance, such as intraoral appliances with and without skeletal anchorage. However, even these devices can produce undesirable tipping of the maxillary molars and/or loss of anterior anchorage during distalization . In the last decades, increasing numbers of adult patients have sought orthodontic treatment and expressed a desire for esthetic and comfortable alternatives to conventional fixed appliances. Invisalign is an orthodontic system that has been introduced to answer this request. Standardized protocols have been developed in order to obtain predictable results with this kind of approach.

Invisalign aligners: from basic research to clinical application - In recent years aligner orthodontics has grown very fast since an increasing number of patients have sought esthetic and comfortable alternative to fixed appliances.
The application of an orthodontic force produces a tissue reaction that is the result of the perturbation generated by the orthodontic appliance and the modeling and remodeling of the alveolar bone.
Aligners provide intermittent orthodontic forces and despite their spread among the orthodontic community, there are still doubt relating to the bone metabolism induced by this kind of appliances.
Researches conducted at the University of Torino, Italy, demonstrated that aligners seem to be capable of inducing the same biological responses described for fixed appliances, at least in the very early stages of the orthodontic treatment. On the basis of this consideration biomechanics of aligner orthodontics should be optimized in order to permit the control of complex movements like the bodily movement of maxillary molars. Results of studies describing the dentoskeletal effects of Class II correction with invisalign aligners will be then presented as well as the effects of the attachments and elastics on the quality of the orthodontic tooth movement.
The effects of the aligners are not limited to teeth only: the occlusal splint effect could be very useful during orthodontic treatment of patients suffering from sleep bruxism. Results from a research on this topic will be discussed.
Tommaso is a specialist orthodontist working in his private practices located in Torino and Neive
and a post-graduate tutor at the Specialization School of Orthodontics of the Lingotto Dental School
of the University of Torino.
He obtained his DDS degree from the University of Torino, Italy, in 1998 and the specialization in
Orthodontics from the same University in 2001. He completed the PhD program in Human
Anatomy at the University of Milan, Italy, in 2007.
He is an adjunct professor of the Chair of Orthodontics at the University of Torino and of the
Specialization School of Orthodontics of the same University since 2012.
Dr. Tommaso Castroflorio is a Clinical Speaker and a key opinion leader for Invisalign, OrthoAccel
and BruxOff.He has lectured in Italy and abroad and serves as reviewer for the most prestigious
scientific journals in the fields of Orthodontics and Oral Physiology. Tommaso has written
numerous papers in orthodontic and oral physiology literatures.
He obtained several Awards from the Italian Society of Orthodontics and from the Italian College
of Dental Teachers for research protocols conducted in the fields of orthodontics and
temporomandibular disorders. In 2014 he won the International Aligner Research Award.
He is a member of the European Advisory Board of Align Technology.
He is a founding member of the European Aligner Society.
He is an active member of the Italian Society of Orthodontics.
His private practice in the offices of Torino and Neive is limited to orthodontics and orofacial pain.
